How our evolving services align with Fit for the Future: the NHS’ 10 Year Health Plan for England
- Family support workers
- Social work service
- Hospital partnerships
- Bereavement support

“The plan recognises what families facing serious illness have always known – that good care can’t only happen in hospitals. It needs to reach homes, communities, and the everyday realities families live through.
At Momentum Children’s Charity, this is exactly the direction our work has taken. Over the past year, we’ve transitioned to a model that places Family Support Workers in both hospital and community settings, bridging the gap between clinical care and emotional wellbeing. This ensures that support is there not just at crisis points, but across the entire journey – before, during and after treatment.”
Why holistic, emotional support matters
“NHS teams play a critical role in treating the medical aspects of a child’s illness – but families need more. They need help navigating the emotional strain, the upheaval on daily life, and the uncertainty that can shadow their child’s condition.
Momentum provides personalised support that includes a consistent, trusted professional family support presence from diagnosis onwards; support with practical pressures – including finances, education and benefits delivered though our Social Work service; respite and experiences that give families the chance to reconnect and feel ‘normal’; and ongoing support after treatment ends.”
Bereavement support for as long as is needed
“When the worst happens and a child dies, families face a depth of grief that doesn’t follow timelines or fixed stages. Bereavement support must recognise this – it must be sensitive, flexible, and rooted in the realities of each family’s experience.
Our Echoes bereavement service provides dedicated support for families, offering a lifeline during the most painful of times. We know that grief doesn’t end after a few sessions or a single follow-up call and that is why our support continues for as long as a family needs it and is shaped by what feels right to them.”
A shared direction
“In conclusion, we welcome the NHS’ commitment to a future that sees emotional and practical support as part of the bigger picture. The Fit for the Future plan recognises the importance of early intervention, integrated care, and localised delivery – values we embed in our everyday work.
As the health system evolves, we will continue to work closely with NHS teams and local partners to make sure families receive the care and support they need – not just during treatment, but throughout every part of their journey.”
