Social work service
We are proud to offer a specialist social work provision at Momentum to help our families who require an enhanced level of advice and practical support.

Ways we can help
Our social worker will work with you one-to-one to understand your personal situation and help support you.

Financial guidance
Money worries can feel overwhelming. We can help you explore grants and welfare benefits, identify support available for debt concerns, and assist with Disability Living Allowance (DLA), Personal Independent Payments (PIP) and Blue Badge applications.

Housing guidance
If your child’s diagnosis has affected your housing needs, such as requiring adaptations or more suitable accommodation, we can provide information, guidance and support to help you understand the options available to you.

Education support
We can advise on education for children with health needs, and liaise with your child’s school, where helpful, to ensure their needs are understood and the right support is in place.

Workplace guidance
We can write a letter of support to your employer, and signpost you to specialist employment support if you need further guidance.

Advocacy
Sometimes it helps to have someone in your corner. We can advocate on your behalf with professionals, local authorities and other organisations to help ensure your voice is heard.

Signposting to more support
If you need support beyond what we can provide directly, we can connect you with other relevant services and organisations that may be able to help.

“Hana, Momentum’s Social Worker, helped us complete our DLA form. She was brilliant, explaining everything in detail. Without Hana’s support, I don’t think we would’ve completed the form – it was a huge task, and we just didn’t have the head space for it.”
